The 2024 World Index of Healthcare Innovation (WIHI) highlights a mix of steady leadership and significant upward shifts among European nations. After a one-year hiatus in 2023, the report confirms that while the top tier remains exclusive, the broader landscape is evolving. Here’s a look at the top countries for healthcare, according to FREOPP.

7. United States

In the 2024 World Index of Healthcare Innovation, the United States made a significant recovery, climbing to 7th place overall and snapping a multi-year streak of decline.

While the U.S. continues to struggle with cost, its rise in the rankings was driven by high-tech leadership and better pandemic positioning.

  • Scientific Dominance: The U.S. remains the global leader in Science & Technology, outperforming second-ranked Switzerland by a wide margin (78.64 vs. 62.25). This is largely due to its high volume of medical patents and new drug approvals.
  • Pandemic Turnaround: The most dramatic improvement was in Pandemic Preparedness and Response, where the U.S. jumped from 23rd in 2022 to 7th in 2024.
  • Choice & Accessibility: The U.S. rose to 6th place in the Choice dimension. This was primarily fueled by the increased availability of biosimilar drugs, which offer more affordable alternatives to expensive biologics.

Methodology

The World Index of Healthcare Innovation (WIHI), established in 2020, stands as the most thorough assessment of healthcare systems among high-income nations. Unlike many standard metrics that focus solely on the OECD, WIHI broadens the scope to include high-performing Asian and Middle Eastern economies.

To provide a balanced view, the index evaluates each country across four distinct dimensions:

  • Quality: Assessing patient outcomes and infrastructure.

  • Choice: Measuring the freedom patients have to choose their providers and treatments.

  • Science & Technology: Evaluating medical R&D and digital health integration.

  • Fiscal Sustainability: Analyzing the long-term economic health of the national system.

For a country to be included in the Index, it must meet two specific thresholds:

  1. Population: Over 5 million people.

  2. Wealth: High GDP per capita based on Purchasing Power Parity (PPP).
